I agree to release Lauren Tansel from any and all claims. I acknowledge that Lauren Tansel is not responsible for any injuries, pain, or illness that may occur as a result of my participation. I agree to disclose any medical or health conditions to Lauren Tansel and take full responsibility for consulting a medical practitioner before participating.
